The fees below are referred to as "Third Party Cash Advance Items". We do not profit from these items, but these services are necessary for us to fulfill our service commitment to you. You need not make payment to these entities - we will, and each charge will be listed on the right side of the "Statement of Goods and Services Selected" that we will provide you when completing Cremation Service arrangements. Only payment to City Funeral Service is required. For your convenience, we accept all forms of Major Credit Card, or Debit.
City Funeral Service prefers to engage with The Historic Woodlawn Cemetery in The Bronx as our Crematory of preference. Woodlawn will perform this delicate process, and the fee listed here includes forwarding of the ashes to you or designated party using the US Postal Service. If you prefer we use another Crematory, please tell us when finalizing services so that we may quote you the correct price. All Souls Crematory (East Elmhurst, Queens), Fresh Pond Crematory (Middle Village, Queens), Ferncliff Cemetery (Westchester) and Greenwood Cemetery (Brooklyn) are reputable organizations in New York offering Cremation to Funeral Directors.
Cremation Permit: $40.00/NYC - $20.00/NY State
The Cremation Permit issued by the New York City Department of Health and Mental Hygiene (DOHMH) when the death takes place in the 5 Boroughs. Beyond New York City, on Long Island or in Westchester, Rockland and Putnam County, this document is issued by the Registrar of Vital Statistics in the City, Town or Village where the death ocurred. This document will accompany the Deceased to the Crematory
Certified Copies of the Death Certificate (Each): $15.00/NYC - $10.00/NY State
Certified Copies of the Certificate of Death bear the Seal of the City of New York, or Municipality where the death has taken place. The copies are used for Insurance Claims, Probate, Property Transfer, or Banking and Financial Transactions. The number of copies needed is determined by the size of the Deceased's Estate. You do not have to order any, but we recommend you order one for your records. The City of New York will provide three (3) copies at no charge if the Deceased is an Honorably Discharged Veteran of the US Armed Forces. However, please keep in mind City Funeral Service DOES NOT issue any copies free, nor are we a registrar. We merely order the copies for you on your behalf, as a convenience to you
NYC eVital Usage Fee: $6.75
Not too long ago, Funeral Directors obtained a Certificate of Death from a Physician or Medical Examiner on a Handwritten Paper Form. We then submitted this to the Department of Health, in person, at their office. As New York City Government finally enters the 21st Century, Funeral Directors and Physicians must use eVital, an internet based system for completing and submitting Certificates of Death to the NYC DOHMH. The fee for system utilization is listed to the right. The customer will receive the Certified Copies on special paper from NYC DOHMH
UPS Delivery Fee: $17.00
This fee ONLY applies to persons who pass away in the 5 Boroughs of New York City (The Bronx, Brooklyn, Queens, Manhattan and Staten Island). The Certified Copies of the Certificate of Death are forwarded to you using United Parcel Service. The fee is listed here
This simple fee of $10.00 goes towards tolls, parking fees, and assistance by Hospital or Medical Examiner staff, and Crematory Personnel

Peter, the owner, took care of both my mom and dad after their passing. Very professional and would recommend.
No, I am not employee. My dad died and I called a number of funeral homes. These guys (Peter Frisolone in particular) were straightforward, responsive and completely fair. They did what they said. I would absolutely recommend their services to make a difficult period of your life (the death of a loved one) go as well as you can hope.
